Right pneumothorax occurred 2 days after laparoscopic-assisted vaginal hysterectomy. The mechanism was most likely CO
2 diffusion from pneumoperitoneum through a diaphragmatic defect. It is essential to be aware of this rare complication because of its late onset after the surgical procedure.
