•Le système lymphatique absorbe la totalité de l’ultrafiltrat des capillaires sanguins contrairement à la compréhension simplifiée de la loi de Starling enseignée jusqu’alors.•Le système lymphatique est ainsi la voie de retour unique de l’absorption du liquide interstitiel et de son transport dans la circulation sanguine.•Les progrès de la biologie moléculaire ont permis l’identification d’une quarantaine de gènes à ce jour à l’origine de lymphœdèmes primaires•La lymphangiogenèse est directement impliquée dans l’adipogenèse des lymphœdèmes primaires et secondaires et de l’obésité
