The common ADA/EASD consensus regarding the management of type 2 diabetes has just been updated. The SFD Statement on the Management of Hyperglycemia in Diabetic Patients of type 2 was published in October 2017. These recommendations agree on the therapeutic objectives in terms of reduction of complications and quality of life, on the importance of the person-centered approach. They are based on the results of several cardiovascular safety studies. The presence of a cardiovascular history must be a major criterion for the choice of antidiabetic treatment, as well as the presence of chronic kidney insufficiency. Thus, the criterion of choice of treatment is no longer limited to cardiovascular safety, but is oriented towards cardiovascular protection. The choice of the therapeutic class, or even sometimes of the molecule will be decided according to the profile of the diabetic person. The ADA/EASD consensus offers us a therapeutic strategy very different from the previous recommendations, justified by the clinical benefit provided by SGLT2 inhibitors and GLP-1 receptor agonists. The treatment is no longer limited to a goal of lowering HbA1c.
