Deep pelvic endometriosis is a common and important gynecologic disorder that is responsible for severe pelvic pain and is defined as subperitoneal invasion that exceeds 5 mm in depth. MRI is the best imaging modality to stage the disease that can affect posterior, medial or anterior compartment of the pelvis. The aim of this review is to describe the different lesions of deep pelvic endometriosis, to underline the key points that must be present in a MRI report and especially to clarify the impact of each lesion on the management of patients with deep pelvic endometriosis.
