The Comparison of Outcomes of Antibiotic Drugs and Appendectomy (CODA) collaborative trial recently reported that antibiotics were non-inferior to appendectomy, based upon their criterion of EQ-5D score at 30 days [1]. The question of relative efficacy of medical treatment for appendicitis is not new [2], and we would like to reflect on several aspects of this high-stake recurrent topic that has courted controversy for more than 60 years.
