•CLIPPERS is a relapsing-remitting brainstem disease of unknown origin•Sentinel lesions of primary CNS lymphoma could fit all features of CLIPPERS•Including our patient, three cases of CLIPPERS progressed to CNS B-cell lymphoma•CLIPPERS and CNS lymphoma are two distinct diseases or belong to the same disorder?•Further investigations are needed to determine the pathophysiology of CLIPPERS
