Zolpidem is a hypnotic drug related to benzodiazepines, used for the treatment of insomnia.It is the most commonly prescribed benzodiazepine among persons over 65 years of age.Due to problems of drug dependence, misuse and even diversion for criminal use, itsprescription is subject to some of the regulatory framework applied for narcotics since April10, 2017. The primary objective of this study was to measure the impact of the reform on apatients population under long-term Zolpidem (> 4 weeks) in a center of general medicalpractice. The secondary objective was to highlight possible transfers of prescriptions onother molecules for the treatment of insomnia.Materials and methods: a monocentric, retrospective and descriptive study of drugprescriptions before and after the reform, of a patient population consulting in the medicalcenter of Sainte Marie la Mer, which was done by extracting data from computerized patientrecords.Results: 128 patients were included in the study. 78 patients (60.9% CI 95% 52.4 - 69.4)maintained the Zolpidem treatment and 50 (39.1% CI 95% 30.6 - 47.9 no longer had aZolpidem prescription 6 months after the reform.44% of patients did not receive anysubstitutive treatment. The most used substitution treatments were for Zopiclone andLormetazepam for 28% of patients, and 12% for Lormetazepam. No non-pharmacologicalinterventions was found in the panel.Conclusion: The reform of prescription conditions of Zolpidem has proved effective inreducing the number of prescriptions. But will the fundamental problem (dependence,misuse and their consequences) not just be moved to other molecules? It would beinteresting to promote the use of non-drug interventions for the treatment of insomnia.They currently offer the best benefit / risk ratio.
