Faced with the over-diagnosis of allergic hypersensitivities to beta-lactams, label removal strategies have recently emerged. Especially for patients requiring emergency BL. The objective of this study is to compare their safety and unnecessary avoidance of BL.The study included patients from the allergology unit of Montpellier University Hospital between 2014 and 2021 with a complete allergological assessment. Patients were independently stratified by 2 assessors into risk groups by applying three algorithms Blumenthal et al., 2015, Romano et al./ENDA 2020 and Trubiano et al., 2020 (PEN-FAST).On a first part of 1,171 analyzed patients, 33% had a positive assessment. These algorithms had a failure rate in terms of safety in 7.9% (Blumenthal), 0.3% (ENDA) and 10.6% (PEN-FAST) of patients.The Romano/ENDA algorithm was the safest. The low-risk category PEN-FAST had the best sensitivity.
