Background: pre-eclampsia (PE) is a pregnancy syndrome defined by a high blood pressure (≥140/90 mmHg) and at least one other associated complication, including proteinuria, maternal organ dysfunction or uteroplacental dysfunction, after 20 weeks of gestation. Long-term renal effects of this disease are not still fully understood. The present systematic review aims to evaluate the risk of chronic kidney disease after pre-eclampsia and the determinants of these sequelae.Methods: from PubMed, Web of Science and Cochrane databases, we included observational cohorts studied kidney outcome in women with a history of pre-eclampsia.Results: on the 277 evaluated, 31 studies (published between 2000 and 2022) were included in this systematic review. 22 authors focused on the risk of CKD and/or persistent proteinuria. 5 found a more significant risk of kidney disease after PE, with a risk ranging from 2 to 3 times. Regarding ESRD, most of studies found an increased risk between 4 to 14 for developing ESRD. Early PE, preterm delivery and recurrent pregnancies complicated by PE appeared as the 3 major factors listed by several authors as determining the risk of renal sequelae or proteinuria.Conclusion: women with a history of pre-eclampsia have an increased risk of CKD or ESRD. Some determinants seem to increase the risk. More research is needed to identify these determinants of renal sequelae.
