Résumé
There are several techniques used for the treatment of the proximal femoral fracture, type 31A of the AO Classification. Those techniques have evolved rapidly in the past decades.Currently, there are three techniques used by the surgeons: the three cannulated, the DHS type plate system and intra-medullary nailing. In this piece we will compare those three techniques one to another to show the which are the advantages of the nailing over the two other techniques. Moreover, the nailing technique has also been some target to innovations. Among those innovation, the anti-rotation system has an important role. However, despite some proof from the literature indicating the superiority of the double lag screw configuration compared to all other techniques currently used, the double lag screw nailing has some difficulties to implement in France. In order to understand why the double lag screw nailing is unpopular in France, a qualitative study have been conducted over traumatology-reconstruction surgeons and residents. The first point that has been identify is the absence of independence of the trauma specialty compared to recon and an absence of interest for the traumatology by the French surgeons.Nonetheless, all the interviewed surgeons have agreed that the treatment of a proximal femoral fracture for a young healthy patient should be treated by nailing. This consensus shows some possibility for the trauma marketing department to improve their market share.
