The quantification of the blood loss for a patient with traumatic injuries allows a pronostic quantification and leads to initiate appropriate therapeutic measures. The ATLS classification of hemorrhagic shock which is based, in part, on hemodynamic parameters is perfectible. Looking for reproducible non invasive tools which have good correlation with blood loss is a necessity. An experimental model of controlled hemorrhagic shock enables us to have hemodynamic and cardiac ultrasonographic measures during a controlled bleeding. Outcomes: the main outcome was to show the correlation between ShockIndex (SI) and volume of bleeding (VOB) in an experimental piglet model of controlled hemorrhagic shock. Secondary outcomes were to study correlation between values of blood pressure (BP) and VOB; between SIand cardiac output (CO) and finally, between pulse pressure (PP) and left ventricle systolic ejection volume (SEV) in that same model. Methods: we analyzed data from sixty-six anesthetized and ventilated piglets who were bled at 2 mL.kg-1.min-1 until obtainin game an arterial pressure (MAP) of 40 mmHg. During this hemorrhagic stage, hemodynamic and cardiac ultrasonographic measures were regularly performed. Results: the correlation coefficient between SIand VOB is 0.70IC95%, [0.64; 0.75] (p<0.01), and this between MAP and VOB is-0.47IC95% [-0.55; 0.38] (p<0.01). Lastly, the correlation coefficients between SIand CO, and between PP and SEV are respectively -0.45IC95% [-0.53; -0.37] (p < 0.01) and 0.62 IC95% [0.56; 0.67] (p < 0.01). Conclusion: in a piglet model of controlled hemorrhagic shock, the correlation between SIand VOB seems strong.
