The pharmaceutical interview and a stronger city-hospital link help to improve adherence to medication. The prospective observational study was conducted, between February and November 2023, in the hepato-gastroenterology department at Montpellier University Hospital. The main objective was to set up a targeted pharmaceutical interview in hospital with liver transplant patients. The interview was carried out on the basis of data from the patient needs survey. The satisfaction of patients and their pharmacy was collected using questionnaires. Anti-rejection drug information sheets and a summary advice sheet for liver transplant patients were drawn up. 27 patients were included in our study, 20 of whom benefited from the pharmaceutical interview. 96% of patients initially wanted to benefit from an interview. Priority was given to drug administration, anti-rejection monitoring and management of critical situations (missed doses). A summary of the interview was sent to the pharmacy in 85% of cases. It was systematically incorporated into the hospital and pharmacy patient file. All patients were at least satisfied (75% very satisfied) with the pharmaceutical interview. According to the pharmacy, the summary saved time and improved professional practice. Their knowledge and comfort with anti-rejection advice was considered unsatisfactory. The documentation created was sent to them. The value of an interview and of strengthening the town-hospital link to secure the care of liver transplant patients was demonstrated.
