In women with genetic predisposition, it is recommended to perform a bilateral prophylactic mastectomy. Nipple sparing mastectomy (NSM) has become the technique of choice. Robotic surgery has been largely developed in nipple sparing mastectomy but there is still problems of cost and accessibility. The aim of this study was to assess the feasibility of a new technique of endoscopic prophylactic nipple-sparing mastectomy (MRI MoP) using a monotrocart, without robotic assistance, by a single axillary incision, with immediate prepectoral prosthetic breast reconstruction (IPBR). Methods: this monocentric prospective study included women with breast cup from A to C and moderate ptosis. The primary endpoint was the rate of complete NSM with IPBR using MRI MoP technique. The secondary endpoints were complications rate and aesthetic results (Breast-Q questionnaire). Results: from April 2019 to October 2021, 8 patients were included (7 bilateral mastectomies and 1 unilateral). The follow up time was 1 year. All the interventions were performed according to MRI-MoP technique. The rate of complication requiring a second surgery was 13% (1 skin necrosis and 1 infection). There was no case of implant loss. Seroma rate was 7%. All patients would choose the same surgical technique and were very satisfied or satisfied with aesthetic results. Lipofilling was necessary in 63% of the patients. Conclusion: preliminary outcomes attest feasibility and safety of this technique. However, long term data and larger cohort are needed to confirm these results.
