La contraception chez les femmes atteintes d’une maladie inflammatoire chronique intestinale : une revue de la littérature

It has been shown that women with chronic intestinal inflammatory disease (IBD) should plan their pregnancy to avoid serious complications. However, there is a lack of French recommendations on the optimal contraceptive choice for these patients. The objective of this thesis was to determine which contraception is most suitable for women with IBD. Method: a literature review was conducted using the “Pubmed” and “Science Direct” databases, including all studies without date restrictions. An in depth analysis was conducted for the 18 articles that meet our selection criteria. Results: oral contraception, in particular combined, seems to increase the risk of relapse of IBD, thromboembolic accidents and cervical dysplasia. The absorption of oral contraceptives would not be decreased and the use of anti-JAK does not interfere with their pharmacokinetics. The intrauterine system may exacerbate Crohn’s disease. No studies have been found on other forms of contraception. Discussion: estrogen-progestin and oral progestin contraception are not recommended. Long-acting contraceptives are recommended as a priority. More studies are needed to optimally guide women with IBD in their contraceptive choices.
