Background: the aBSI (automatic Bone Scan Index) is an automated, reproducible, rapid and simple tool for quantifying bone damage, which could enable a more detailed analysis of the progress of patients undergoing treatment, particularly in relation to PCWG3 criteria.Methods: we collected aBSI on follow-up scans of hormone-sensitive or hormoneresistant patients with metastatic prostate cancer treated with Docetaxel. We studied the impact on survival of aBSI at a given time and of its variation over time.Results: 58 castration-resistant and 19 castration-sensitive patients were included. Survival of resistant patients with crude aBSI <5% at first post-Docetaxel scan was significantly shorter than patients with aBSI >= 5% (HR=2.96; (CI: 1.05-8.32) p<0.001); and tended to be shorter in patients whose relative change in aBSI at the second post Docetaxel scan was >= 15% (p=0.039; HR=1.92; CI: [0.98-3.79]). Sensitive patients whose relative change in aBSI at first post-Docetaxel reassessment was greater than -42% also tended to have shorter survival (p=0.04; HR=2.91; (CI: 0.93-9.06)).Conclusion: this study shows the prognostic value of aBSI at post-treatment reassessment. A trend towards prognostic significance of its variation over time was also found, although contradictory data in the literature encourages further investigation.
