According to WHO data, climate change is projected to be responsible for an increase of over 250,000 annual deaths by 2050. In France, approximately 41 million pressurized metered-dose inhalers (pMDIs) are sold each year, for chronic respiratory diseases (such as asthma and COPD). Pressurized MDIs containing hydrofluoroalkane emit 15 to 30 times more greenhouse gases than other inhalers. The primary objective of this pilot study was to assess the reduction in the prescription of pMDIs. Method: this was an open-label, non-randomized, non-controlled, multicenter interventional pilot study. The calculated number of subjects was 29. The study population consisted of general practitioners working in primary care practices or group practices. Recruitment took place from March 2023 to October 2023 via email. Participants received a 40-minute educational training on prescribing lowcarbon impact inhalers. The primary objective was assessed using a questionnaire. Results: we recruited 34 participants. There was a significant decrease in the total prescription of pMDIs from 34% to 4%. In reliver therapy, the prescription decreased from 71% to 7%. Participants significantly improved the quality of their prescriptions by adhering to GINA recommendations in both reliver therapy (6% vs. 38%) and maintenance therapy (35% vs. 56%). However, this effect was not observed in COPD. 93% of participants reported being "very satisfied" with the presentation, and 80% stated that they had "changed their prescription habits". After the intervention, 40% reported considering ecological factors in their prescriptions. Discussion: this pilot study has demonstrated its effectiveness in reducing the reported prescription of pressurized MDIs with a clear outcome measure. It holds significant educational value by validating three out of four levels of Kirkpatrick's model. The main limitations lie in the selfreported nature of the primary outcome measure, the lack of a control group, and the dropouts of 4 participants. This study will serve as a foundation for the implementation of interventions on a larger scale.
