Environmental sustainability in anesthesia: reintroducing reusable laryngoscope blades

The climate crisis represents a major threat to global health. Current healthcare systems significantly contribute to the carbon footprint. Operating rooms are responsible for a large share of greenhouse gas emissions, particularly due to the use of single-use equipment, which generates waste. Transitioning to reusable equipment is a sustainable solution. This study aims to assess user satisfaction following the transition from single-use laryngoscope blades to reusable laryngoscope blades (RUB) in Nîmes University Hospital operating rooms (OR). Methods: we conducted a prospective before-after study in the OR of Nîmes University Hospital. Two satisfaction surveys were administered to anesthesiologists, nurse anesthetists (IADE), and IADE students who had used RUB, at 1 months and 5 months after their introduction. The primary endpoint was users’satisfaction with RUB. The secondary endpoint evaluated intubation quality. Results: at 1 month, 70% of users reported being generally satisfied with RUB use. Satisfaction rates were comparable at 5 months (67%). However, a non-significant but increasing proportion of users reported being slightly or not satisfied. Intubation quality was deemed equivalent in the majority of cases, but 25% of users rated it as inferior. The main reasons for dissatisfaction were related to the larger size of RUB, which hindered intubation. Numerous logistical challenges were also reported. Conclusion: while RUBs are perceived as environmentally beneficial, logistical and technical challenges have been identified. Further efforts are needed to improve user adherence and optimize the management of reusable equipment.
