Implantation of an interatrial shunt device reduces pulmonary capillary wedge pressure and may improve symptoms in patients with heart failure. This retrospective, single-centre, cohort study analyzed the medical records of 19 patients who had been implanted with an AFR interatrial shunt device between June 2021 and May 2024 at Montpellier University Hospital (France). We sought to identify predictive factors for response to interatrial shunt therapy. A major cardiovascular event (defined as a composite of cardiovascular death, heart failure events and heart transplant) occurred in 11 (58%) patients. Resting PCWP at baseline was significantly higher in patients who had a major cardiovascular event compared with those who did not (p=0.02). NYHA functional class, quality of life (KCCQ-OS) and functional capacity (6MWD) tended to improve at 3 and 12 months, without reaching statistical significance. Implantation of the device was safe in all patients
