Aneurysmal subarachnoid hemorrhage has an elevated death rate and leads to severe disability. Anxiety and depression symptoms are often reported among patients and prevent them to resume their professional and social activities. Studies show that insula, amygdala and frontal cortex are involved in the development of anxiety but there is currently no link between cerebral injuries due to SAH and the development of psychiatric disorders. This retrospective observational study relates patients admitted for aneurysmal SAH in the year 2021. We conducted teleconsultations to evaluate the neurologic functional outcome and the prevalence of anxiety and depression symptoms at 6 months. We analyzed localization and volume of cortex infarction among patients with anxiety symptoms. One hundred thirteen patients were included. Twenty-two patients died. We were able to carry out teleconsultation for 74 of the 91 survivors. Fifty-eight (60.4%) patients had good outcome. The prevalence of anxiety and depression symptoms were respectively 50% (95%CI 39-62%) and 36.5% (95%CI 31-49%). Twenty-one percent of patients resumed their professional activity without any change. Eleven anxious patients presented cerebral injuries on scans. Three patients showed lesions in areas potentially linked to anxiety. This study shows that after SAH half of the patients presents anxiety symptoms and more than a third suffers from depression symptoms. These psychiatric disorders have a great negative impact on the return to professional activity. The presence of cerebral injuries on standard imaging does not seem to be linked to the onset of anxiety symptoms.
