Résumé

Interventions pharmaceutiques Pharmacie d’officine Act-IP Officine Échelle CLEO
The mission of pharmacists revolves around a fundamental and mandatory activity: medication dispensing. This pharmaceutical activity involves analyzing, dispensing, refusing, and optimizing medication prescriptions with Evidence Based Medicine. The objective of our work was to analyze 200 pharmaceutical interventions in a community pharmacy in France using the Act-IP© Officine tool. The results of our analysis firstly demonstrate the feasibility and potential documentation of pharmaceutical interventions in a community pharmacy setting. These results also highlight the importance of analyzing such interventions for improving individual and collective practices. Thus, in this thesis, we describe the typology of various pharmaceutical interventions performed and measure their clinical, economic, and organizational impacts. These measurements are carried out by the community pharmacy team as well as external clinical pharmacists. The anticipated outcomes of analyzing the entire dataset of pharmaceutical interventions captured in the Act-IP© Officine database offer significant prospects in quantifying and qualifying the pharmaceutical activity.
