A program for the reduction of HIV-1 mother to child transmission based on nevirapine single dose prophylaxis has been going on at the MCC of the CBF since January 2000. Babies are included from birth till 18 months, for biological and clinical followed up, initially on monthly basis for the 6 first months and quarterly thereafter. 136 babies have been included so far. 103 have had a viral load assessment at 6weeks. 13 were infected, 18 had uncertain results and 72 were not infected. The rate of transmission is comprised between 7.2 and 20.1% at 95% C.I. There was no significant difference between mothers who had infected babies and those who had not, concerning CD4 count (p=0.5) and premature rupture of membranes (p=0.29). The following clinical features were significantly different: recurrent fever during pregnancy (p=0.045), thrush (p=0.0005), skin disease (p=0.00007), respiratory infection (p=0.0024). These could indicate a higher proportion of severe cases.
